I have read, acknowledge, and agree to the following:  

• I understand and agree that in participating in any dance class or rehearsal, there is a possibility of physical injury. 

• I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me during any dance class conducted by Jess Cohan. 

• I understand that I should be aware of my physical limitations and agree not to exceed them. 

• Jess Cohan will not be held responsible for any personal or property damage during any time of our transaction and I will relinquish Jess Cohan and the owners of the location of any responsibility or monetary compensation. 

• I agree to obey the facility rules and take full responsibility for my behaviour in addition to any damage I may cause to the facilities utilized by Jess Cohan.  

• I agree that no food or beverages are allowed in the studio, except water.  

• I understand that refunds will not be permitted once classes/services are purchased.  Any purchased private classes can be used for up to 1 year.

• Clean shoes must be worn in the studio.  For heels classes, please ensure your heels do not leave marks or have sharp edges that could damage flooring. To avoid this, it is recommended to duct tape the bottom of your heels.  

 

PHOTOGRAPHY/VIDEO – I voluntarily agree, and therefore grant permission for Jess Cohan to use photographs or videos of me for publicity purposes.  

• If I post any video content/choreography on social media, I understand to tag Jess Cohan (IG: @jess.moves and/or @jcodechoreo) for choreography credit.  

• I agree not to use any of the choreography taught by Jess Cohan at any time, for any purpose, without permission.
